West Parry Sound Health Centre (WPSHC) has launched a Patient and Social Services Navigator Role to better serve patients, families, caregivers, and all citizens in our community. Additionally, this new role will strengthen interconnectivity between the health centre and other health and social service partners in the region.
The health care system can be complex for patients and families to navigate, as each situation is unique, and each patients has different care needs. The primary role of the Patient and Social Services Navigator is to ease the burden of navigating by providing effective and efficient guidance to those in need.
WPSHC is very pleased to share that Bobbi-Jo Henry has accepted the new role and has been involved with community partners to define how navigation services can beneficially support those receiving and providing care. A long-time employee at WPSHC, Bobbi-Jo brings a thorough understanding of the internal programs and services offered at the health centre, and as a recently certified Patient Navigator she is also equipped with the knowledge needed to drive this new role from the conceptual stage to fruition. Having worked in the role for several months, she is excited to officially launch the role to the public on March 5th. She is now available to assist patients, families, community members, and our health and social service partners, through a ‘no-wrong door’ approach and can be reached at 705-773-8496.
“The health centre is very pleased to support the launch of a Patient and Social Services Navigator,” said Donald Sanderson, Chief Executive Officer for WPSHC. “ We are confident that this role will create positive impacts within our communities. While Bobbi-Jo is located within the health centre, we see this role as a community resource available to our partners and citizens.”
“Bobbi-Jo will be supporting patients and families within the health centre,” said Victoria Langley, Director of Nursing and Clinical Services, “however, much of her work will be focused on following up and providing support to patients after they have been discharged. She will ensure that they continue to have access to the supports they need and will be able to connect individuals with services such as counselling, mental health supports, disability programs, exercise programs, and community services such as transportation and meals services.”
Community members who just don’t know where to turn and are need of assistance are encouraged to reach out for guidance by calling 705-773-8496 or emailing bhenry@wpshc.com.
