										<content:encoded><![CDATA[<p>The Couchiching Ontario Health Team (OHT) was proud to announce this past February, that it had been officially acknowledged by the Ministry of Health (MOH) and Ontario Health (OH) for its outstanding proposal to expand interdisciplinary primary care team services to its community. This recognition highlights the OHT&#8217;s commitment to improving <em>access to care</em> and enabling access to the <em>right care, in the right place</em> for patients who otherwise do not have access to a healthcare provider.</p>
<p>As health system partners, the Couchiching OHT has played a crucial role in fostering collaboration, patient and caregiver co-design and integration within the healthcare system. By embracing an integrated co-design approach, the Couchiching OHT is dedicated to enhancing the overall health and well-being of the community it serves.</p>
<p>As part of this primary care expansion initiative, the COHT, in collaboration with the Couchiching Family Health Team (CFHT) and the Huronia Nurse Practitioner-Led Clinic (HNPLC) operational teams, will be broadening their interdisciplinary primary care teams to better cater to the needs of the community. In the upcoming weeks and months, the CFHT will offer supplementary support for allied health services and programs, while also extending primary care services to both attached and unattached patients in the area. Simultaneously, the HNPLC will expand its services with a focus on paneling 1,000 patients, ensuring attachment for those who are most medically complex and in need within the community.</p>
<p>In light of these positive expansions, it is with mixed emotions that OHT announces the closure of the COHT-OSMH COVID Care Clinic. This clinic, born out of COVID operations, has been instrumental in the community&#8217;s pandemic response and recovery efforts for nearly four years. OHT extends their heartfelt gratitude to the Care Clinic team and all staff who have worked tirelessly in collaboration with them during this pivotal time in the community&#8217;s history. The COHT-OSMH COVID Care Clinic will cease operations effective June 21, 2024, with the team actively ensuring continuity of care for current patients during the winding-down process.</p>
<p>In the coming weeks, the CFHT and HNPLC will ramp up their operations, beginning in June. The COHT will disseminate information regarding access to care initiatives through its social media channels and newsletters. <p class="xparagraph"><span lang="EN-US">Starting this September and throughout the academic year, a nurse practitioner will be located at the University’s 500 University Avenue campus for one day each week to see students for episodic care or to see those without a primary care provider. As part of the formalized service agreement, students can also receive care at the Couchiching OHT Care Clinic located at OSMH.  </span><span lang="EN-US"> </span></p>
<p class="xparagraph"><span lang="EN-US">“Knowing where and when to access healthcare services can be a significant stressor for students, particularly those attending from out of country,” said Cheryl D’Angelo, Director of Student Health and Wellness at Lakehead University. “This unique partnership is welcomed by students and their families.”</span></p>
<p class="xparagraph"><span lang="EN-US">“Having accessible student-focused health care on campus aims to meet the needs of many students who do not have access to primary healthcare providers within our community,” said Dr. Linda Rodenburg, Interim Principal of Lakehead’s Orillia Campus. “It is another reason why students will choose Lakehead in the future.”</span><span lang="EN-US"> </span></p>
<p class="xparagraph"><span lang="EN-US">Students will be able to access non-emergency healthcare services for prevention and wellness as well as injury and illness starting September 14.</span></p>
<p class="xparagraph"><span lang="EN-US">“This new initiative aligns strongly with the Shared Purpose of the Couchiching OHT and the Hospital – <i>We are a community that is committed to improving health and wellness</i>,” said Melanie Moore, Director of Integrated Planning at OSMH and Co-Director of the COHT. “Strengthening our partnership with Lakehead through the delivery of integrated health services for students is an excellent example of how we are transforming health services within our community.”</span></p>
