Share your child’s Immunization record with the health unit

In Ontario, under the Immunization of School Pupils Act, health units are required to have up to date immunization records for all students.

If you do not provide this information to your health unit, your child could be temporarily suspended from school.*

Check your child’s yellow immunization card and remember to share their immunization record with your health unit. If your child has missed any of their vaccinations, contact your health unit.

Sharing your child’s immunization record with your health unit is required to ensure everyone is protected.

Some parents of high school students may have received an immunization letter from the health unit that contained a printing error on the reverse; if this pertains to you, please disregard. The immunizations listed on the front of the letter are what we need to update your child’s record.
